1. Clinical manifestations
Pain characteristics: rest pain, rest pain, can be reduced after activity, severe cases can wake up in sleep, need to get out of bed after activity can fall back to sleep.
Swelling and pain in large joints (hip, knee, ankle) of the lower extremities: the first symptom in half of patients, asymmetric arthritis.
The spine is rigid from bottom to top, first the lumbar lordosis disappears, and then the kyphosis deformity and limited movement of the cervical spine; Thoracic breathing range may be reduced. Late stages are often associated with fractures.
2. Signs
Sacroiliac joint tenderness; limited forward flexion, posterior extension, scoliosis, and rotation of the spine; Decreased chest range of motion (< 2.5 cm); abnormal occipital wall distance (>0 cm); positive Schober test (< 4 cm); positive 4 test suggests sacroiliac joint disease.
